About the FPA

What is the FPA?

The Faculty Practice Associates (FPA) includes more than 800 physicians and surgeons and is an integral part of the Mount Sinai School of Medicine.

All FPA members are practicing physicians who are also full-time faculty members of the Mount Sinai School of Medicine. In addition to their faculty positions, all FPA members hold medical staff privileges at the Mount Sinai Hospital.

Most FPA physicians see patients in their offices on the campus of the Mount Sinai Medical Center at 5 East 98th Street in New York City. Some FPA physicians have offices in lower Manhattan (Chinatown), on Staten Island, in Astoria (Queens), White Plains (Westchester), and Englewood, New Jersey.

Are all the members of FPA specialists?

The FPA includes both primary care physicians and specialists of all kinds.

What if I don't have a primary care physician?

You can find a selection of primary care physicians here by searching our Find a Doctor directory.

Do I need to see a specialist?

A primary care physician is the gateway to all specialty services at the Mount Sinai Medical Center and should be consulted first. If you don’t have a PC P, or if you haven't had a checkup lately, it may be time to make an appointment, during which you can discuss your options. Please use our convenient Find a Doctor search box to locate a primary care physician matched to your needs.

I need laboratory tests. Can they be performed at the FPA?

Yes. The FPA provides comprehensive services for laboratory tests, health care screening, diagnostic testing, radiology, and other imaging procedures. These diagnostic services will be ordered by your physician.

How can I register a complaint or make a comment about the care I received at Mount Sinai FPA?

How do I decide which specialty practice to contact?

Billing and Insurance

Who can provide answers for my billing questions?

Please call Patient Financial Services for the appropriate department during regular business hours 9:00-5:00. Patient Financial Services telephone numbers for specific patient care areas are listed at right.

Why did I receive a letter requesting my insurance information?

Your claim may be missing information, or may have been incorrectly completed. To avoid a claim rejection by your insurance carrier, we will contact you to obtain the correct information so that your claim may be quickly filed and processed.

After I see a physician, has a claim been filed with my insurance company on my behalf?

Yes. A claim is automatically sent to your insurance company based upon the information you provided to the registration staff at the time of your visit.

How can I find out if insurance has paid my bill?

After your claim has been processed, your insurance company may send you an explanation of benefits (EOB). You can also contact your insurance provider directly. The provider’s telephone number can usually be found on your insurance identification card. In addition, most insurance companies maintain Web sites for their insured members. Once registered on the site, you may access your claim information, download insurance policy coverage documents and insurance forms, determine which physicians participate in your plan, and other useful information.

Why am I receiving bills from different physicians?

Each physician who provides professional services will bill you separately.

My primary physician has referred me to a specialist. What should I do with the referral document?

You should bring your referral to the specialist and give it to the receptionist at the front desk. This referral is required before your claim will be paid by your insurance company.

Why did my insurance company pay only part of my bill?

Most insurance plans require you to pay a deductible and/or co-insurance or co-pay. Also, your insurance plan will not pay for services not covered under your benefit plan.

How can I pay my patient balance?

You may pay by credit card, cash, check, or money order at the time of your visit. Payments for balances due may be sent to the address found on the billing statement that you receive.

Why did the hospital and my doctor send me separate bills?

The hospital and physicians bill separately for their services in accordance with health insurance regulations.

I received a statement the other day. How often will I receive this statement?

You will receive a statement each month until your bill is paid in full.

Why am I getting a bill for pathology services?

Your physician may have performed a biopsy, obtained a Pap smear, or taken another type of lab specimen during your office visit. Although you may never have visited Mount Sinai Medical Center, your specimens were sent to our laboratories for processing and interpretation. The FPA physician bills you for his or her service rendered as part of the referral process. By New York State law, fees for laboratory work must be billed to you directly by the laboratory that provides the services.

My deductible under Medicare has been met. Why am I receiving a bill?

If you do not have secondary or supplementary insurance in addition to Medicare, you are being billed for the balance. If you have supplementary insurance, that plan may also have a deductible.

How do I know if my insurance plan will cover certain services?

Coverage varies with each carrier, and in accordance with your insurance contract. Please refer to your member’s handbook or call your insurance carrier with your questions.

Which insurance plans do FPA doctors accept?

FPA physicians participate in the majority of plans offered by various insurance carriers. However, individual physicians accept some plans of a particular carrier but not others. For details, please contact your doctor’s office.

What charges will my insurance cover?

Your insurance company may cover the principal charges that you incur. However, you are responsible for deductibles and co-payments. Anesthesiology services and certain laboratory tests and procedures also may not be covered. For a more comprehensive discussion, see our Brief Guide to Insurance and Managed Care.

I don't have insurance. What should I do?

Contact your doctor's office. Trained personnel will work with you to find a solution. There may be services available in a hospital-based setting on a sliding fee scale.

How do I find out whether my anesthesiologist participates with my insurance plan?

Although anesthesiologists at Mount Sinai participate with many managed care organizations, they do not participate with all carriers. We encourage you to be aware of the specifics of your own policy and to contact us at (800) 627-4470 if you have questions.

What if anesthesiologists do not participate in my insurance plan?

If you would like an estimate of anesthesiology charges, please contact us at (212) 427-2666 or (800) 627-4470. We will need to know the name of your surgeon and the type of operation planned. To ensure an accurate estimate, please ask your surgeon to provide the procedure, or CPT, code for the planned surgery as well as the diagnosis, or ICD-9 code.

If you contact us 7-10 days before your surgery, we may be able to determine to what extent your plan will cover your anesthesiology charges and what your out-of-pocket expenses may be, if any. Unfortunately, in some cases, carriers will not communicate with us. In those cases, our staff will suggest you telephone your carrier yourself and our staff will tell you what questions to ask.

What if I cannot afford to pay my medical bill?

For charges not covered by insurance, monthly payment arrangements can be made if you cannot afford to pay your entire balance at one time. Mount Sinai has a policy to assist patients who face financial hardship. To qualify for assistance, you must supply documentation as requested.
